We would like to hear from you about where we should spent the donations we receive. In the past we have purchased mattresses for the dialysis unit, sheds for CAPD patients, blood pressure monitors for home dialysis patients & much more.

To request a grant, you will need to complete a form requesting financial assistance and give it to your Renal Social worker. If he/she is unable to assist, your letter/form will be given to the LKPA. Your request will be reviewed at the next LKPA Committee meeting.
